Participants are kindly requested to wear their badges during all congress activities and social events. For full delegate, the badge will admit to the Opening Ceremony, Scientific Sessions, exhibition areas and coffee breaks. Any request for replacement of a lost badge must submit proof of original registration payment, and subject to a handling fee of US$30.

CME Credit Points
 
The 3rd APCH has been approved for CME credit points by the Singapore Medical Council

Certificate of Attendance 
 
A certificate of Attendance will be issued on request to all registered participants during the Congress.
 
II. Registration
You may submit your registration via regular mail or via the internet. Registration Form without payment will not be accepted. Registration fee must be paid for the abstract to be published.

III. Confirmation
 
Your registration will be confirmed by the Congress Secretariat within 3 to 4 weeks after receipt of the Registration Form and the necessary payment. Confirmation of registration will be issued by email or fax only. Please include your email address and fax number in the form.
 
IV. Amendments
 
No amendments will be accepted over the telephone or fax. Please post or email any amendments to the Congress Secretariat. No amendments will be accepted after 15 Feb 2004.
 
V. Cancellation and Refunds
 
 
1.
All cancellations must be submitted in writing to the Congress Secretariat before 31 Dec 2003. Fax copy is not allowed
   
2.
Only Registration fees will be refunded less US$90 per person for cancellation received on or before 31 Dec 2003. 
   
3.
There will be no refund for any cancellation received after 31 Dec 2003. 
 
VI. Payments of Fee
1.
All prices are quoted in US Dollars. Payment in other currencies will not be acceptable. The fees can be paid by credit cards or bank draft. Please note that personal or company cheques cannot be accepted. Payment of fees must accompany all Registration Forms. The registration will be authorized only after receipt of the registration fee.

Bank Draft:

Bank drafts will need to be drawn on a Singapore Bank only. Singapore Banks include Development Bank of Singapore, Overseas Chinese Banking Corporation, United Overseas Bank, Overseas Union Bank, Standard Chartered Bank (Singapore Branch) and Citibank (Singapore Branch).

Should the bank draft not be drawn on a Singapore Bank, then an administrative charge of US$35.00 will be imposed for each bank draft issued to the 3rd APCH.

Bank draft should be made payable to “3rd Asian-Pacific Congress of Hypertension” (please follow exactly what is indicated here). Bank drafts must be paid in US Dollar. Please indicate clearly your name and address on the reverse side of the bank draft. Please mailed the bank draft together with the completed Official Registration Form and Hotel Form to:

Credit Card  
 
Only VISA, MasterCard.or American Exress Card are acceptable. The cardholder’s name, card number, expiration date of the card and an original signature from the cardholder should be filled in on the Registration Form
 
VII. Official Letter of Invitation
 
1.
Individual who wishes to receive an official letter of invitation to the Congress must apply to the Congress Secretariat Office and include brief summary of reason of request.

Please note that this applies to individuals requiring a visa or other special forms of permission to attend the Congress. A letter of invitation does not commit the Congress defraying registration fees, transportation, hotel accommodation or any other expenses incurred by participants.
