Please note that abstracts will only be considered for presentation after receipt of the Registration Form and Registration Fee.

I. SUBMISSION OF ABSTRACT VIA REGULAR MAIL
 
1.
 Simply download the Abstract Submission Form from PDF File or cut out the form in the 3rd APCH Announcement
   
2.
Complete Page A1 of the Abstract Submission Form using typewriter or block letters.
   
3.
Abstracts to be saved on a floppy disk with 3 photocopies of the completed Abstract Submission Form
   
4.
Download the Registration Form PDF file and Hotel Booking Form from PDF file or cut out the necessary forms in the 3rd APCH Announcement
   
5.
Payments for Registration and Hotel 1-night advanced deposit by bank draft or credit card. For credit card, do not send by fax.
   
6.
Send all the above materials to:
